Preservation First. Extraction When It's the Right Call.

Dr. Gonzales's starting point is always the same: find a way to keep the tooth. Extraction enters the conversation only after restoration options have been genuinely evaluated and a clear clinical reason exists to move in a different direction.

When a tooth has reached the point where saving it would cause more harm than removing it, you'll know why before anything is decided. What we're seeing, what it means for your surrounding bone and teeth, and what your options are. Nothing moves forward without that clarity.

An extraction may be the right call when:

  • Decay has reached the pulp, and a root canal is no longer a viable option
  • Infection has progressed to the point where it's threatening the surrounding bone or neighboring teeth
  • Periodontal disease has caused enough bone loss that the tooth can no longer be supported
  • A crack or fracture runs below the gumline, and the tooth cannot be restored
  • A wisdom tooth is impacted, causing pain, pressure, or damage to the tooth next to it
  • A failing tooth is causing ongoing inflammation that's affecting your overall oral health

If you're not sure whether your tooth can be saved, that's exactly the kind of question we work through together before making any decision.

What Happens During a
Tooth Extraction at Opal Dental Studio

Every extraction at Opal follows the same principle: understand the full picture first, move carefully, and make sure you know what's happening at every step.

Step 1: A Full Evaluation Before Anything Else

Dr. Gonzales reviews your medical history, current medications, and anything that could affect your healing. She takes digital X-rays and, when the anatomy calls for it, 3D cone beam imaging to see the tooth, bone, and surrounding structures in detail. She also takes one more look at whether the tooth can still be saved before the extraction conversation moves forward.

Step 2: Comfort Planning

Dr. Gonzales uses a local anesthetic for every extraction. If you want to feel calmer during the procedure, nitrous oxide is available. Mild conscious sedation is an option when it fits your situation. She walks you through each step before she begins, so nothing feels unexpected.

Step 3: The Extraction

For a simple extraction, Dr. Gonzales gently loosens the tooth and removes it with precision instruments. When a tooth is impacted, fractured at the gumline, or hasn't fully erupted, she makes a small incision to access it properly. She chooses the approach based on your anatomy rather than what's fastest.

Step 4: Socket Preservation

When it makes clinical sense, Dr. Gonzales places bone grafting material in the socket during the same visit. This maintains jawbone volume while the area heals and keeps your future options intact, especially if you're considering a dental implant later. It's built into the plan from the start.

Step 5: Recovery and What Comes Next

We send you home with clear post-op instructions and an explanation of what's normal versus what needs a call. We put a follow-up plan in place based on your case. Dr. Abbey will explain the importance of tooth replacement and walk you through your options. When you're ready to move forward, you set the pace.

Simple Extractions vs. Surgical Extractions:

The type of extraction depends on where the tooth sits and what it takes to safely remove it.

A simple extraction is appropriate when the tooth is fully erupted and visible above the gumline. Dr. Gonzales loosens it and removes it without making an incision.

A surgical extraction is used when the tooth is impacted, broken at or below the gumline, or positioned in a way that requires accessing the surrounding bone or gum tissue. Wisdom teeth frequently fall into this category, depending on how they've developed and where they sit in the jaw.

Dr. Gonzales handles both in-office. When a case is complex enough to require an oral surgeon, she coordinates the referral directly and remains involved in your care throughout.

Wisdom tooth removal is available in-office for most cases. Dr. Gonzales
reviews your imaging and gives you a straight answer about what your
situation calls for.

Wisdom Tooth Removal
in Huntersville, NC

Wisdom teeth, your third molars, typically emerge between 17 and 25. Because of where they sit in the mouth, they're often difficult to clean properly with brushing and flossing, which makes them more prone to decay and gum issues over time.

Whether removal makes sense depends on how they've developed and what they're doing to the surrounding teeth and bone.

Dr. Gonzales evaluates the position, eruption status, and effect on neighboring teeth before making any decision. Removal comes into the conversation when a wisdom tooth is impacted. An impacted wisdom tooth puts pressure on the tooth next to it, triggering repeated infections around the gum tissue. They can also contribute to crowding that affects your bite.

Before anything else, we’ll take cone beam 3D imaging to see root shape, angle, and proximity to nearby nerve structures. That detail matters for a safe, predictable procedure.

What to Expect After Your Extraction

Most patients feel well enough to return to normal activity within two to three days for non-surgical extractions. The first 24 hours matter most for setting up a smooth recovery.

The first day:

○ Bite down gently on gauze to control bleeding and allow a clot to form
○ Apply an ice pack to the outside of your face in short intervals to manage swelling
○ Stick to soft foods: soups, yogurt, eggs, mashed foods
○ Avoid rinsing, spitting forcefully, or using a straw, all of which can disturb the clot

In the days that follow:

○ Rinse gently with warm salt water after meals, starting the second day
○ Continue soft foods until the area feels comfortable enough to reintroduce others
○ No smoking or tobacco, which significantly slows healing and increases the risk of a dry socket
○ Keep brushing and flossing, but stay away from the extraction site until it has closed

Call the office if you experience:

○ Severe or worsening pain after the second day, which can signal dry socket
○ Fever, increasing swelling, or discharge around the site
○ Bleeding that doesn't settle with gentle pressure after an hour

Dr. Gonzales sends you home with written instructions specific to your case. Nothing is left to memory.

After the Extraction:
What Comes Next

A missing tooth does more over time than just leave a gap.

  • The surrounding teeth begin to shift.
  • The jawbone in that area starts to shrink without a root to support it.
  • Bite mechanics change, sometimes in ways you don't notice until later.

This is why Dr. Gonzales starts the replacement conversation at the extraction appointment. Not to push a decision, but so you leave with a clear picture of your options and what timing looks like.

Depending on your situation and goals, that conversation may include a dental implant, an implant-supported bridge, or a partial denture. Socket preservation, often placed at the time of extraction, supports all of these by maintaining the bone volume you'll need.

The timeline is yours.
